- Provide a demographic/geographic overview of Colerain Township
- Discuss the role and creation of the Colerain’s Community Health Coordinator (CHC)
- Discuss the importance for addressing psychosocial needs in addition to addiction and recovery resources.
- Provide an overview of the CHC’s job description and explain the role within both QRT and the community referral program.
- Discuss the operational data and performance criteria used to evaluate program efficacy

- Serve as expert/advisor for Substance Use Disorder/Addiction
- QRT (Quick Response Team)
- Work with Fire/EMS & PD to provide care coordination & link individuals to resources
- Assist with second chance employment programs by working with businesses/chamber of commerce
- Partner with Northwest School district to provide prevention/intervention for first time drug and alcohol offenders Overall Job Description
- Team goes into community every Thursday morning to follow up with individuals who have experienced an overdose
- Includes intentional and unintentional overdose incidents
- Includes any substance use, not just opioids
QRT
- Team consists of CHC (Community Health Coordinator), Paramedic, and Police Officer
- Offers individuals treatment resources and harm reduction supplies/resources (Narcan, fentanyl test strips)
- Addresses any other additional needs that may be present
- Provides bags to people in need that include food, water, and basic hygiene items
- Morphed into more than just addiction/recovery focused – follow-ups done with any community member that has been identified as needing assistance
- Data for each client is entered into Cordata QRT Data 2021:
Total Overdose Responses – 146 QRT Overdose Follow Ups – 101 (69%) of total overdoses. These were the individuals we determined were Colerain residents.
Face to Face Connections – 69 (68%)
Engaged in Treatment – 42 (61%)
- Partnered with both the fire department and police department in Colerain
- Fire/EMS refers clients online through their electronic documenting system
- PD refers clients via paper reports CHC Referral Program
- Eight different categories first responders can refer someone to:
- Addiction services
- Mental health services
- Domestic violence/abuse
- Environmental services
- Food, clothing, rent, or utility assistance (basic needs)
- Medical services
- Senior services
- Veteran services.
- If needed, CHC will go on scene with first responders to assist in real time
- Ensures there is open communication with first responders in order to update them on the outcome of each referral
- All client data entered into Cordata 2021:
- Received 352 referrals from fire/EMS and the police
- 303 (86.1%) of the referrals were eligible clients (live in Colerain Township)
- CHC was able to contact 183 (60.4%) of the clients
- 118 clients (61.2%) were referred to services
- The top three most common referral types were addiction services with 23.97%, medical services with 19.07%, and mental health services with 15.72% CHC Referral Program Data
- Work with NWLSD students who have been charged with minor, first time drug/alcohol offenses on school grounds
- Parents reach out to schedule a time to meet with the CHC and student when student is found to be using or have drugs/alcohol in their possession at school
- NWLSD requires the student to take a drug test and meet with the CHC in order to avoid prolonged suspension or possible expulsion
- CHC completes a short screening interview with the student, called the CRAFFT interview (https://crafft.org/wp-content/uploads/2021/07/CRAFFT_2.1_Clinician_2021-07-03.pdf)
- Provides the family with resources for the student, including drug/alcohol treatment, mental health treatment, or alcohol/drug education classes.
- CHC met with 46 students who were referred from NWLSD during the 2021-2022 school year Northwest Local School District Partnership Moving Forward
